You can find important forms and documents relating to your insurance policy in this library. We recommend using Adobe Reader to open these PDF files. Please note that you won't be able to edit forms that require a penned signature – you'll need to print them to fill them in.

Payment matters
If you're setting up GIRO, please check if your policy number is in either of the following formats:
- P9999999999 – where it starts with P, followed by 10 digits
- 99999999 – where there are 8 digits and does not start with 0
If your policy number is in either of those formats, please use this interbank GIRO form (PDF, 638.8KB).
If it's not in either of those formats, please use this interbank GIRO form (PDF, 409.9KB) instead.
To report payments or top-up amounts either:
- sourced outside Singapore
- of SGD12,000 or more
Please use a premium payment form (PDF, 559.8KB).
Nomination of beneficiaries
Online Nomination of Beneficiaries (Adobe Live Sign)
Important notes for online nomination of beneficiaries:
- For online nomination, you and your witnesses/attestors are required to attach a copy of NRIC, passport or other valid identification document.
- All information including personal information and identification documents will be visible to all parties.
- If you and/or your witnesses/attestors are not agreeable with (i) attaching personal identification document to the electronic nomination form or (ii) other parties having access to your personal information and identification document, please do not proceed with online nomination. Hardcopy nomination is available above.
- You need to have a valid registered email address with HSBC Life. To update your email address, please complete the personal particulars update form available here.

FATCA and CRS
If you need to complete a Foreign Account Tax Compliance Art (FATCA) form or a Common Reporting Standard (CRS) form, you can find the form you need here. CRS is equivalent to a tax residency self-certification form.
